New Patient Registration

We accept new patients that reside in the practice area.

To register at the practice patients should attend the Medical Practice during surgery hours and complete a registration form.

The practice area is mainly North East Telford, especially the areas of Donnington, Trench, Muxton, Wrockwardine Wood and St. Georges. Please use the map and postcode checker tool below to see if you live within our area.

If you wish to pre-register click on the links below to open the forms. When you have completed all of the details, click on the “Submit” button to mail your form to us.

Non-urgent advice: Please note

When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days.

After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP.

After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment.

You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Disabled Patient Facilities

Disabled patient facilities are also available at this practice.

Emergency Services | Служби надзвичайних ситуацій

Call 999 if someone is seriously ill or injured, and their life is at risk. An ambulance will come to help.

If you have a medical emergency, we have two large hospitals: one hospital in Shrewsbury and one in Telford. Our Accident and Emergency departments (A & E) are open 24/7 all year, and you don’t need an appointment. Sometimes you will be admitted to hospital to continue your care.

If you have seriously harmed yourself or are about to do so, call 999 or go to hospital. If you are thinking about suicide call the charity Samaritans on 116 123 for support. Mental Health Shropshire also offers a hotline and support groups across Shropshire, Telford and Wrekin – call 01743 368647.

You don’t always need to go to a big hospital. We have Minor Injury Units (MIUs) across Shropshire and Telford which help with cuts, broken bones, sprains and infections.

Refugees with young children

For refugees arriving with young children, please use our local health visitor service.

Health visitors offer support and advice to families on a variety of topics including child weight, breastfeeding, supporting new mothers with post-natal depression, baby weaning, parenting programmes and putting parents in touch with a variety of specialist services that can support them and their child’s healthy development.
